OFFICE UPDATE AS OF 01/04/2021

Dear Rockenmacher Chiropractic Family,

We hope this email finds you well and that you are all staying healthy in these current times. We just wanted to give you a quick update about the office.

The office is open with a limited schedule. To schedule an appointment. you can call the office at (562) 598-9609. As part of an effort to increase the sanitation and disinfectant processes of the office, the office will be disinfected using an EPA Registered Ultra Violet light as well as a surface disinfectant. The appointments will continue to be scheduled in 30-minute intervals to allow for proper sanitation and reduced contact between patients.

In addition to the increase of the office's health and safety practices, we are adopting new health and safety practices as it relates to the patients and their treatment. We will be requiring that every person that enters the office to wear a face mask. If you are not wearing a face mask, the office has a limited supply to be sold and will be sold for $2.00 each. Additionally, each patient may purchase electrode pads for their treatment of electrical stimulation, if applicable to the patient's treatment.

Please notify the office when scheduling an appointment if you have: 

• Been experiencing symptoms related to COVID-19 (i.e. fever, loss of smell or taste, coughing, muscle ache, sore throat, congestion or a runny nose, shortness of breath, chills, headaches, nausea)
• Been in close contact with, been diagnosed with COVID-19, or placed in quarantine for possible exposure to COVID-19 within the last two weeks
• Been asked to self-isolate or quarantine by a medical professional or a local public health official in the last two weeks
• Travelled within the U.S. or any foreign country within the last 14 days

Upon arrival for your appointment, you will be asked to complete the following form to verify the aforementioned information. For your convenience, you may complete the form and email it back to us or bring a completed copy to your appointment.

COVID-19 Screening Questionnaire

We urge all of our patients to continue practicing the following health safety practices as suggested by health officials such as the WHO and the CDC:

• When in public, wear a face mask or cloth face covering over your nose and mouth
• Practice social distancing by maintaining 6 feet between you and another person
• Wash your hands frequently with soap and water for at least 20 seconds, especially after using the restroom, blowing your nose, and before you eat
• Avoid touching your eyes, nose, and mouth with unwashed hands
• Cover your coughs and sneezes using your elbow or a tissue

• Clean and disinfect objects and surfaces regularly
• Stay at home if you’re feeling sick and take precautions when around those who are sick
